Community Aged Care Packages (CACPs)
A Commonwealth funded program, to assist the frail aged and those with dementia who with appropraite support by a low level care service can remain living at home.The service is flexible and coordinated to meet the individual's assessed needs. Services can include:-
- Personal care assistance
- Domestic assistance
- Meal preparation
- Medication monitoring
- Assistance with shopping/banking/bill payment
- Transport to and from medical appointments
- Advocacy, case management.
Services are flexible to adapt to the changing needs of the client. Individuals are supported by a coordinator who manages their needs and allocates care staff to assist them. The coordinator will provide 'case management' support which is in addition to the direct service received in the home.
The elegibility requirement for CACP Packages is an assessment by the Aged Care Assessment Team (ACAT). This is a free assessment that will determine if a CACP program is the appropriate level of care for the individual. Assessments can be arranged through Carrington or by contacting your local area ACAT.
