Extended Aged Care at Home (EACH) General and Dementia Packages
A Commonwealth funded program to assist the frail aged and those with dementia who with appropriate support by a high level care service can remain living at home.The service is flexible and coordinated to meet the individual's assessed needs. Services can include:-
- Nursing
- Personal care
- Domestic assistance
- In-home respite
- Transport
- Social support
- Advocacy and case management
The elegibility requirement for EACH Packages is an assessment by the Aged Care Assessment Team (ACAT). This is a free assessment that will determine if an EACH program is the appropriate level of care for the individual. Assessments can be arranged through Carrington or by contacting your local area ACAT.
